This article will provide information on the steps you need to take to change your Pag-IBIG membership or employer information, as well as the requirements you must fulfill to make these changes.
How To Change Pag-IBIG Membership Information
What Are the Valid IDs Accepted by the Pag-IBIG Fund?
Regardless of the type of information you’ll change or update, Pag-IBIG will always request a valid ID.
You must submit one photocopy and present the valid ID’s original copy for authentication purposes

- Passport
- SSS ID/UMID
- GSIS e-Card

Note: The Pag-IBIG Fund doesn’t recognize PhilHealth ID and TIN ID as valid.
Pag IBIG Change of Information Online: A Guide for Registered Individual Members
To modify or update their membership information, Pag-IBIG members need to obtain a Member’s Change of Information Form (MCIF). This form can be obtained at the Pag-IBIG office or downloaded and completed in advance by visiting the provided link or downloading it from their website.

Keep these in mind when filling out the MCIF:
You are required to complete only one copy of the form. Fill in only the relevant sections that need to be changed or updated. For instance, if you want to change your membership category from employed to voluntary, then fill in the section under “Change of Membership Category” only. Leave the other fields empty or indicate N/A to show that they do not apply to your request. Use BLOCK or CAPITAL LETTERS when answering. Refer to the following sections for instructions on changing specific details in your Pag-IBIG membership record. The form must be filled in the English language.

The form must be filled out in English.
a. Change of Membership Category
- Obtain the latest version of the Member’s Change of Information Form (MCIF).
- Write your Pag-IBIG MID number in the top right corner of the form.
- Tick the box labeled “Change of Membership Category.”
- Fill in your full name (last name, first name, middle name, and name extension) in the name field.
- In the “1. Change of Membership Category” section, indicate your current category in the “FROM” field and the category you wish to move to in the “TO” field (e.g., FROM: Employed, TO: Self-employed).
- Sign your name above your printed name in the “Certification” section and include the transaction date.
Submit the completed form with one photocopy of a valid ID to the nearest Pag-IBIG office.
b.Change/Correction of Name
If you need to correct or update your name in your Pag-IBIG membership record, you can do so by following these steps:
- Obtain the Member’s Change of Information Form (MCIF).
- Write your Pag-IBIG MID number in the upper right corner of the form.
- Tick the “Change/Correction of Name” box.
- Provide your complete name (last name, first name, name extension, and middle name) in the name field.
- In section 2, indicate the specific part of your name that needs to be changed/corrected in the “FROM” field and the correct form in the “TO” field. For example, if your current name is “de la Cruz” and you want to change it to “Dela Cruz,” you should write “FROM: de la Cruz, TO: Dela Cruz.” If you recently got married and want to include your maiden name, you can also indicate it in the “FROM” field and your husband’s surname in the “TO” field.
- Sign and date the form in the “Certification” section.
Visit the nearest Pag-IBIG office and submit the completed form with one photocopy of a valid ID and one photocopy of a supporting document. If you changed your name due to marriage, you can bring your marriage contract. For name changes not related to marriage, you can bring your birth certificate and, if applicable, the Court Order granting the petition of change of name.
c.Correction of Date of Birth
- Obtain the latest copy of the Member’s Change of Information Form (MCIF).
- Write your Pag-IBIG MID number on the upper right corner of the form.
- Check the box for “Correction of Date of Birth.”
- Fill out your complete name (last name, first name, name extension, and middle name) in the name field.
- Under “Correction of Date of Birth,” indicate your current date of birth on record in the “FROM” field and your correct date of birth in the “TO” field.
- Sign over your printed name in the “Certification” section and indicate the date of the transaction.
Submit the completed form with one photocopy of your valid ID and one photocopy of your birth certificate to the nearest Pag-IBIG office. Ensure that your birth certificate shows your correct date of birth legibly; otherwise, your request may be rejected.
d.Change of Marital Status
Civil status can be modified due to various reasons such as death, annulment, marriage, or incorrect data encoding. If you need to update your marital status on your Pag-IBIG membership record, follow these simple steps:
- Obtain the latest copy of the Member’s Change of Information Form (MCIF).
- Write your Pag-IBIG MID number in the upper right corner of the form.
- Check the box that corresponds to “Change of Marital Status.”
- Complete the name field with your full name (last name, first name, name extension, and middle name).
- Under “4. Change of Marital Status,” indicate your current civil status in the “FROM” field and your new or correct marital status in the “TO” field (e.g., FROM: Single, TO: Married). For married women, you can choose to use your husband’s surname, a hyphenated surname (maiden name – husband’s surname), or retain your maiden name. Tick the box that corresponds to your preferred surname and make the necessary changes to your registered name (refer to “b. Change/Correction of Name” above). Lastly, if you’re a married member, you’ll also need to provide your spouse’s full name and date of birth.
- Sign the “Certification” section with your signature over your printed name. Indicate the transaction date.
- Go to the nearest Pag-IBIG office and submit the completed form along with one photocopy of your valid ID and one photocopy of any of the following supporting documents: Marriage Contract (for Single to Married); Court Order (for Married to Single, applicable for members whose marriages have been annulled); CENOMAR (for Married to Single, applicable for members whose civil status has been changed due to a clerical error); and Death Certificate of the deceased spouse (for Married to Widowed).
By following these steps, you can update your Pag-IBIG membership record with your correct marital status.
e.Change of Address/Contact Details (Cellphone Number, Landline Number, or Email Address)
To update or correct their address, phone number, or email address, Pag-IBIG members need to complete the following steps:
- Obtain a copy of the latest Member’s Change of Information Form (MCIF).
- Write their Pag-IBIG MID number in the upper right corner of the form.
- Check the box corresponding to “Change of Address/Contact Details.”
- Fill out the name field with their complete name (last name, first name, name extension, and middle name).
- Under “5. Change of Address/Contact Details,” only fill out the section/s they want to be changed or updated. For instance, if they’ve relocated to a new address, they can provide all the necessary information under “Present Home Address.” They can leave other fields blank or write “N/A.” In this section, they also have the option to update their preferred mailing address, cellphone number, and email address.
- Sign over their printed name in the “Certification” section and indicate the transaction date.
Submit the accomplished form with a photocopy of a valid ID to the nearest Pag-IBIG office.
f.Change of Employment Details
If you’re a member of Pag-IBIG and you’ve recently changed jobs or companies, you need to update your membership record with your new employment details. Here’s how you can do it:
- Obtain the latest version of the Member’s Change of Information Form (MCIF).
- Write your Pag-IBIG MID number in the upper right corner of the form.
- Check the box that says “Change of Employment Details”.
- Fill in your complete name (last name, first name, name extension, and middle name) in the name field.
- Under “6. Change of Employment Details”, fill in only the sections you need to change or update. You can update your employer’s name, address, employment date, job title, and employment status.
- Sign your name and write the date of the transaction in the “Certification” section.
Submit the completed form along with a photocopy of a valid ID to your nearest Pag-IBIG office.
g.Updating of Heirs
